Community Stoma Service

Overview

We are specialist nurses in bowel management and stoma care including ileostomy, colostomy and urostomy. We are hosted by Coloplast and work across the hospital and community settings, ensuring patients are cared for throughout their journey.

We work with patients prior to surgery, ensuring they are confident and knowledgeable about their future care. We support patients to adapt to life with a stoma, and give practical help and guidance to the patient, relatives, carers and friends. One of our nurses is also qualified in pediatric stoma care.  

 

Services we offer:

  • Bespoke training for professionals
  • Joint assessments with the community nursing team for complex patients
  • Supply of stoma care products to all community nursing teams
  • Free online training for professionals and patients/ carers 
  • Access to a free stoma care app to support high quality care
  • Home visits to support patients in times of crisis 
  • We attend two patient support groups in the area:
    - Ostomy Support Group held quarterly in the evening
    - Ostomistics held on the 1st Wednesday of every month in the afternoon
