Community Physiotherapy

ECCH’s Community Physiotherapy service is a team of Physiotherapists and Therapy Assistant Practitioners who provide assessment and services to any adult aged 18+ in the Great Yarmouth and Waveney area.

We assess the needs of adults who have physical problems which may be due to injury, illness, or ageing, in their own home. We support patient when they come home from the hospital, prevent unnecessary hospital admissions and support people to stay at home for as long as possible including patients who are receiving end-of-life care.

We provide rehabilitation support in the home setting, to maximise independence and self-management.

 

What is Physiotherapy?

 Physiotherapists assess how you are managing your mobility, balance, strength, and pain. We can advise you on how to improve your physical function and quality of life. We can provide you with exercises, education, and advice to help you achieve your goals.

 

How does this service help you?

We will work closely with you to set and achieve your goals. As part of your therapy programme, we may provide you with some equipment on short or long-term loan to help you move around more easily. We will teach you how to use it safely as part of your therapy programme. Where appropriate, some people will get support with managing their long-term conditions and may be taught breathing techniques, fatigue management, and falls prevention strategies to achieve this.

We can offer assessments in people’s place of residence. We also offer telephone assessments depending upon a patient’s needs.

Our core hours are between 08:30am-4:30pm across all sites (excluding weekends and bank holidays), however we can offer appointments outside of these hours depending on individuals’ needs.
