Cardiac Rehab

Overview

Cardiac rehabilitation is designed to support you following certain recent heart conditions.
Who We are:

Our team consists of Cardiac Rehabilitation Team Leader, Cardiac rehabilitation nurses, Cardiac exercises therapist, Cardiac support worker and Volunteers

What we do:

The cardiac Rehabilitation team helps people through providing you and your family with the information, support and advice you need to get back to everyday life.

With the aim in you looking after yourself to reduce your chances of further heart problems following a heart attack, coronary angioplasty or stent, coronary artery bypass.

The service also helps people to maintain and improve physical fitness if they meet a specific Heart Failure criterion or a Spontaneous Coronary Artery Dissection (SCAD ) or Takotsubo cardiomyopathy


Accessing the service

We currently accept referral for the following conditions

  • People over the age of 18 years of age
  • Registered with a GP within the Great Yarmouth and Waveney and Integrated care system
  • Recent heart attack (myocardial infarction / Non-ST Elevation myocardial infarction or ST Elevation myocardial infarction)
  • Recent coronary angioplasty or stent. (percutaneous coronary intervention or PCI)
  • Recent Coronary Artery bypass surgery (CABG)
  • Recent Spontaneous coronary artery Dissection (SCAD)
  • Recent Takotsubo cardiomyopathy event
  • Stable heart failure with reduced ejection fraction (HFrEF)

Referrals are received directly from the Norfolk and Norwich University Teaching Hospital, James Paget University Hospital and the Royal Papworth Hospital following admission with the relevant heart condition.

Cardiac consultants, GPs, Practise nurses and other health professional may also refer appropriate people who meet the referral criteria.

Self-referrals are not accepted.

You need to be able to attend clinic appointments in the community for the necessary cardiac rehabilitation assessment.

We will contact you by telephone.

This phone call is to acknowledge the referral and arrange a date and time for you to attend your initial assessment. In one of the clinics held at one of the community venues .

Waiting times can sometimes vary by location, so you may be able to get an earlier appointment if you’re able to travel to another site.

Northgate Hospital, Great Yarmouth

Sole Bay Health Centre, Reydon

Stella Maris Hall, Lowestoft

At your clinic appointment, the cardiac rehabilitation nurse will undertake

  • Individual assessment & review of cardiac risk factors
  • Suitability Individual exercise assessment /prescription and monitoring of progress – the full assessment for this may take place or require an additional appointment at a more suitable time dependent on your presentation.
  • Discuss suitability community cardiac rehabilitation exercise / education recovery programme to aid your recovery
  • Advise on Home based walking programme
  • Explain & advise on your heart condition
  • Explain & advise on any heart medication you have been prescribed
  • Offer lifestyle advice and support
  • Signposting to relevant agencies/disciplines
  • Discuss and agree an individual plan
  • You may wish to bring a family member, friend or carer to the appointment if that is helpful to you.
  • We aim to help you build the skills and confidence to manage your condition long term. The more you understand your condition, the more effective your treatment will be. Your clinician is always happy to answer questions, so please feel free to ask!

 

Please bring medication in original boxes and any discharge paperwork to assist the appointment.

Wear suitable clothing & supportive footwear that you can comfortably move around in & are able to roll your sleeve up on your arm to check your blood pressure if it is not short sleeved.

Please bring along any additional aids you may have, such as walking supports and your glasses & hearing aids.

The appointment on average will last approx. 60 – 90 minutes
