Heart Failure

We are a community based heart failure specialist nurse team providing education and support to patients who have a confirmed diagnosis of heart failure in the Great Yarmouth and Waveney area.


What you can expect from our service:


  • Education and information on your diagnosis of heart failure
  • A clinical assessment on each review
  • Lifestyle advice
  • Advice on self-monitoring, how to recognise worsening heart failure symptoms and when to seek help
  • Review of your heart failure medications and information on what they do
  • Assessment of these medicines to achieve an optimum dose for you
  • We work with your GP, Cardiologist and any other healthcare professional involved in your care
  • Emotional support
  • Prevention of hospital admission or readmission where possible
Patients will be discharged back to the care of their GP once stable and on optimised therapy


We provide community clinic appointments, home visits for the housebound and telephone support.


Service times: Monday – Friday, 08:30-16:30


Criteria for referral to this service:
  • The patient is aged 18 years or over
  • We have patient consent
  • Patients have a diagnosis of Left Ventricular Systolic Dysfunction (LVSD) confirmed by an Echocardiogram
  • Patients with non-LVSD heart failure must be under the care or known to a Cardiologist
  • Patients must be registered with a GP in the Great Yarmouth and Waveney area.
