Overview
We are a community-based Diabetes Specialist Nurse team, who provide education and support to patients with diabetes in the Great Yarmouth and Waveney area.
We work closely with local GP practices and have Diabetes Consultant support from the James Paget University Hospital.

- Education and information about your diabetes
- Lifestyle advice
- Full review of your current treatments for diabetes and what they do
- Discussion of potential treatments for your diabetes
Referrals
Criteria for referral to the service includes adults with type 2 diabetes who are registered with a GP in the Great Yarmouth and Waveney area.
Referrals are made via your GP or practice nurse, from the hospital or a healthcare professional.

Service times: Our hours of work are 8:30am-4:30pm, Monday to Friday (excluding public holidays)
