The study aims to look at whether the Support Needs Approach for Patients (SNAP) tool, which was originally developed for patients with COPD, can be used to support older patients more generally.  The plan is therefore that SNAP will be used by District Nurses and the team will collect qualitative data to explore what, if any, benefits it brings to patients and HCPs, and to consider any barriers and facilitators to implementation.
